Obtenez un num\u00e9ro de dossier"},"content":{"rendered":"<p>La Commission de la s\u00e9curit\u00e9 professionnelle et de l\u2019assurance contre les accidents du travail (CSPAAT) peut \u00eatre inform\u00e9e de votre blessure ou de votre accident par\u00a0:<\/p>\n<ul>\n<li>vous-m\u00eame;<\/li>\n<li>votre m\u00e9decin;<\/li>\n<li>votre employeur.<\/li>\n<\/ul>\n<p>Vous pouvez chacun <a href=\"https:\/\/stepstojustice.ca\/fr\/questions\/employment-and-work\/quand-et-comment-dois-je-signaler-une-blessure-ou-un\/\">envoyer un avis de blessure<\/a> \u00e0 la CSPAAT. Si votre employeur ou votre m\u00e9decin signale votre blessure ou votre accident avant vous, la CSPAAT vous enverra une lettre avec un lien vers un formulaire <strong>en ligne<\/strong>. Ce formulaire est <a href=\"https:\/\/stepstojustice.ca\/fr\/resource\/formulaire-d-incident-d-exposition-travailleur-pdie\/\" target=\"_blank\" rel=\"noopener\">Signaler une blessure, une maladie ou un incident d&#8217;exposition<\/a>.<\/p>\n<p data-path-to-node=\"1\">Lorsque vous r\u00e9pondez aux questions en ligne, vous :<\/p>\n<ul>\n<li data-path-to-node=\"2,0,0\">cr\u00e9ez un compte de la CSPAAT, et<\/li>\n<li data-path-to-node=\"2,1,0\">remplissez le Rapport de la l\u00e9sion ou de la maladie professionnelle (travailleur) (Formulaire 6).<\/li>\n<\/ul>\n<p data-path-to-node=\"3\">\u00ab D\u00e9poser \u00bb un formulaire ou un document signifie le transmettre \u00e0 la CSPAAT en suivant les r\u00e8gles \u00e9tablies \u00e0 cet effet.<\/p>\n<p data-path-to-node=\"4\">Dans la semaine suivant le d\u00e9p\u00f4t du Formulaire 6 aupr\u00e8s de la CSPAAT, vous devriez recevoir une lettre de leur part. Si vous ne recevez rien, <a href=\"https:\/\/stepstojustice.ca\/fr\/resource\/wsib-contactez-nous-1-1-1-1\/\" target=\"_blank\" rel=\"noopener\">communiquez avec la CSPAAT<\/a> pour vous informer de l&#8217;\u00e9tat de votre demande de prestations.<\/p>\n<p data-path-to-node=\"5\">Cette lettre contiendra :<\/p>\n<ul>\n<li data-path-to-node=\"6,0,0\">un num\u00e9ro de demande de prestations (num\u00e9ro de dossier)<\/li>\n<li data-path-to-node=\"6,1,0\">le nom d&#8217;une personne-ressource qui s&#8217;occupera de votre dossier<\/li>\n<\/ul>\n<p data-path-to-node=\"7\">Chaque fois que vous communiquerez avec la CSPAAT, on vous demandera votre num\u00e9ro de demande de prestations. Assurez-vous donc de le conserver dans un endroit o\u00f9 vous pourrez le retrouver facilement.<\/p>\n<h3>Gestionnaire de cas<\/h3>\n<p>D\u2019habitude, la personne-ressource est aussi celle qui prend les d\u00e9cisions concernant votre demande et :<\/p>\n<ul>\n<li><a href=\"https:\/\/stepstojustice.ca\/fr\/questions\/employment-and-work\/quels-types-dindemnisation-des-travailleurs-puis-je-obtenir-de-la\/\">les prestations auxquelles vous avez droit<\/a> ou<\/li>\n<li><a href=\"\/fr\/questions\/employment-and-work\/que-dois-je-faire-pour-reprendre-le-travail-apr\u00e8s-ma\/\">votre retour au travail<\/a>.<\/li>\n<\/ul>\n<p>Cette personne s\u2019appelle un gestionnaire de cas.
