Avisez votre conjoint"},"content":{"rendered":"\n<p>Vous n\u2019avez pas besoin de faire approuver votre changement de nom officiel par votre conjoint. Mais vous <strong>pourriez <\/strong>devoir prouver au gouvernement de l\u2019Ontario que vous avez avis\u00e9 votre conjoint de votre demande de changement de nom. C\u2019est ce qu\u2019on appelle \u00ab\u00a0fournir un avis\u00a0\u00bb.\u00a0<\/p>\n<p>Vous devez aviser votre conjoint <strong>seulement <\/strong>si\u00a0:<\/p>\n<ul>\n<li>vous \u00eates mari\u00e9, ou<\/li>\n<li>vous avez sign\u00e9 une d\u00e9claration commune d\u2019union conjugale que vous n\u2019avez pas officiellement annul\u00e9e.<\/li>\n<\/ul>\n<p>Si vous vivez en union de fait, mais que vous n\u2019avez pas sign\u00e9 de d\u00e9claration commune d\u2019union conjugale, vous n\u2019avez <strong>pas<\/strong> besoin d\u2019aviser votre partenaire.<\/p>\n<h3>Comment fournir un avis<\/h3>\n<p>Si vous devez aviser votre conjoint, remplissez la premi\u00e8re partie du formulaire d\u2019avis. C\u2019est la Formule\u00a06, Loi sur le changement de nom, dans la partie\u00a02 de la demande de changement de nom. Votre conjoint doit remplir la deuxi\u00e8me partie et vous la renvoyer. Vous devez alors joindre le formulaire rempli \u00e0 votre demande.<\/p>\n<p>Si vous ne vivez pas avec votre conjoint, envoyez-lui le formulaire d\u2019avis par courrier recommand\u00e9 \u00e0 sa derni\u00e8re adresse de r\u00e9sidence. Quand vous envoyez un document par courrier recommand\u00e9, vous recevez un re\u00e7u. Joignez ce re\u00e7u \u00e0 votre demande.<\/p>\n<p>Si votre partenaire ne signe pas le formulaire d\u2019avis, le re\u00e7u de l\u2019envoi recommand\u00e9 montre que vous avez bien envoy\u00e9 l\u2019avis.<\/p>\n<p>Vous devez aviser votre conjoint du moins <strong>30\u00a0jours avant <\/strong>d\u2019envoyer votre demande.<\/p>\n<p>Si vous vivez avec votre conjoint et qu\u2019il signe le formulaire d\u2019avis, vous n\u2019avez pas besoin de le lui envoyer par la poste.
