2. Give your application to your employer and the Ministry of Labour

I don’t agree with a Ministry of Labour inspector. What can I do?Give a copy of your application and the completed Form C-44 to your employer and the Ministry of Labour. You'll also need to give them a copy of the inspector's report.
You can give these documents to your employer in person, or by fax, regular mail, or courier. If your employer agrees, you can also send them by email.
You can send these documents to the Ministry of Labour by faxing it to 416-326-7985 or mailing them to:

Keep a copy of any receipt as proof that you gave your employer and the Ministry of Labour a copy of your application. You should also note the date and time you gave the application.
Then, on page 7 of Form A-65, write down who you gave the application to and how you gave it to them. If you gave the application by email, you will also need to complete Form A-139 and send it to the OLRB.
If you are not e-filing, you will need to include blank copies of the following forms:
- Form A-66: Response/Intervention – Appeal of Inspector’s Order
- Information Bulletin No. 21: Appeals of an Inspection’s Order – Occupational Health and Safety Act
Keep your original application documents in a safe place in case the OLRB asks for them later.
