1. Choose the right application form
If your landlord has reduced or cut off vital services, care services, or meals, you can apply to the (LTB). To apply, you can download and print the Application about Tenant Rights – Form T2 or fill in and file the form online.
You can also use Form T2 if your landlord has reduced or cut off any other services, like laundry, air conditioning, or internet.
Part 1 of the form asks for basic information about you and your landlord, the address of your rental unit, and the names of any other people affected by your .
If you can't get mail or phone calls at the rental unit, give a mailing address and phone number where you can be reached.
Your landlord might be a person or a company. Make sure you identify your landlord correctly. If you are not sure who your landlord is, you can look at a copy of your rental agreement or any other letters or notices from your landlord.

Form T3
Sometimes it can be impossible or very difficult for your landlord to give you the service back. For example, the phone company might not offer landline service anymore. If this happens or if you don't want the service back, you can a Tenant Application for a Rent Reduction – Form T3 instead of a Form T2. If the LTB agrees with your application, they'll order your landlord to lower your rent.
Form T6
Sometimes a service is cut off or reduced because there are maintenance problems. For example, a furnace that needs repair might not heat your home properly.
If you think you're not getting the service because of repair problems, you should file a Tenant Application about Maintenance – Form T6, instead of a Form T2.
