Guided Pathway for making a human rights complaint
CLEO’s Guided Pathway for making a human rights complaint is for people who have experienced discrimination or harassment in Ontario and want to make a complaint to the Human Rights Tribunal of Ontario. This pathway can help you fill out an Application (Form 1). When you finish the pathway, you can download a Next Steps Guide that explains how to finalize your application and submit it to the Tribunal.
The pathways ask you questions about your situation, give you information to help answer them, and creates documents based on your answers.
The pathways are free to use. When you’re finished, you can save or print your forms. You will also get instructions about how to give them to the Tribunal.
You can make a human rights application if:
- you experienced discrimination in at least one of the five social areas in the Human Rights Code, like employment or housing
- you were discriminated against because of at least one of the grounds of discrimination protected by the Code, like race, sex, or disability
- the discrimination happened to you in Ontario
- the discrimination happened to you within the last year, even if it started earlier
There are some exceptions to when and where the discrimination happened. The pathway will ask you some questions to make sure you can use it to make a human rights application.

Using the pathway
It can take between 1 to 4 hours to complete the pathway, depending on your situation.
You can sign up to create an account or use the pathway as a guest. If you choose not to create an account, the system will time out after 1 hour of inactivity. You must create an account if you want to save your work on the system, or use the other pathways.
