2. Gather information about your finances

If you're not on Ontario Works (OW), start by applying to the Ontario Disability Support Program (ODSP).

You must give detailed information about you and other people in your household to show that you qualify for ODSP.

It's a good idea to collect all the information and documents before you apply. The application process goes faster if you have them ready.

Collect information for you and your household

You need to give ODSP the following information about you and anyone in your household:

• name and date of birth
• immigration status
• income
• assets

You must also give:

• your address
• your housing costs and what type of housing you live in, for example, whether you rent or own
