How do I start an ODSP application?You fill out the (DDP) after the Ontario Disability Support Program (ODSP) decides that you qualify financially.
If you qualify financially, ODSP sends you the DDP.
You will not get the DDP if you're someone who does not have to prove that you have a disability.
If you did not qualify financially
If ODSP decides that you do not qualify financially, you may be able to appeal that decision. For help and advice about appealing, contact a community legal clinic.
You can fill out the DDP while you wait for your appeal. You get the DDP from the ODSP office.
It's important to do this. If you win your appeal, you can start getting ODSP from the date that the got your application.
Fill out the forms
The DDP has 4 forms:
- Health Status Report
- Activities of Daily Living Index
- Consent to Release Medical Information
- Self Report
An approved health professional must fill out the Health Status Report and the Activities of Daily Living Index. They send in these forms online. Or they can fill out a paper form and mail it to the Disability Adjudication Unit.
You must fill out the other 2 forms and send them to the Disability Adjudication Unit. The DDP comes with an envelope addressed to the Disability Adjudication Unit that you can use to mail the forms.

Deadlines
You and the medical professional must fill out and send the forms to the Disability Adjudication Unit within 90 days from the date you got the forms.
If you got the package in the mail, the deadline is 93 days from the mailing date on the envelope.
If you need more time
If you cannot send your forms in on time, contact the Disability Adjudication Unit and ask for more time.
You'll have to prove that you have a good reason. For example, they should give you more time if you could not fill out the forms because you were in the hospital or in jail.
But if they refuse to give you more time and you miss the deadline, you must start your application again.
