Guided Pathway to preparing a power of attorney

CLEO’s Guided Pathways can help you make powers of attorney.

You can use the guided pathways to:

  • Make a Continuing Power of Attorney for Property that gives another person the power to manage your property for you. Property is anything you own or have the sole right to use, like money, a rental apartment, or furniture.
  • Make a Power of Attorney for Personal Care that gives another person the power to make decisions about your personal care, if you can’t make them yourself. Personal care includes things like healthcare, housing, food, clothing, hygiene, and safety.
  • Cancel an old Power of Attorney.

The pathway asks you a series of questions, gives you information to help answer them, and creates documents based on your answers.

The pathway is free to use. When you’re finished, you can save or print your documents. You will also get instructions about how to use these documents.

You can use this pathway if: What you need to get started:
  • you’re 18 years of age or older and making a Power of Attorney for Property, or 16 years of age or older and making a Power of Attorney for Personal Care you live in Ontario
  • the person or people you want to make decisions for you live in Ontario
  • the names and addresses of the person or people you want to make decisions for you
  • the names and addresses of two witnesses
  • the names and addresses of additional people who you want to involve in making decisions
  • the name of a charity, if you want to continue giving to it
  • if you are cancelling an old Power of Attorney, the date the document was created, and the names of the people appointed in it

Using the pathway

It can take as little as 20 minutes to complete the pathway if you have all the information you need to answer the questions. In more complicated situations, it can take up to an hour.

You can sign up to create an account or use the pathway as a guest. If you choose not to create an account, the system will time out after 4 hours. You must create an account if you want to save your work on the system, or use the other pathways.
