I’m a birth parent. Can my adopted child find me?

3. If you want your child to find you or contact you

If you want your child to find you or if you want them contact you in a certain way, you can add your name to the Adoption disclosure register and register a notice of contact preference.

You have to give as much information as you can about yourself and your child, including anything you know about them after the adoption took place. You must give your child's:

  • name
  • place of birth
  • date of birth

Adoption disclosure register

If you've added your name to the adoption disclosure register and your child also adds their name, you may be matched.

To put your name on the register you must fill out a form called an Application to be Named on the Adoption Disclosure Register. It is also called form 3093E. There is no fee to add your name.

For help filling out the application form, look at the Guide to Completing an Application to be Named on the Adoption Disclosure Register

It can take months or even years to be matched. Your child might never put their name on the register.

Notice of contact preference

A notice of contact preference tells your child how you want to be contacted.  For example, you can tell them to contact you by email, telephone, through a third party, or by letters.

To register a notice of contact preference you must fill out a form called an Adoption Information Disclosure Application to Register or Withdraw a Notice of Contact Preference. It is also called form 11314E.

For help filling out a notice of contact preference, look at the Birth Parent’s Guide for Completing and Submitting an Application to Register or Withdraw a Notice of Contact Preference under the Vital Statistics Act.

When you register a notice of contact preference, your child does not have to contact you that way. They may choose another way to contact you.

Changing your mind

You can apply to update your information or change your mind at any time after you have registered on the adoption disclosure registry or given a notice of contact preference.  There is no fee to register or withdraw a notice of contact preference.

To update your information or withdraw from the adoption disclosure registry you have to fill out a form called an Application to Update Information or Remove Name from the Adoption Disclosure Register.

To withdraw a notice of contact preference you must fill out a form called an Adoption Information Disclosure Application to Register or Withdraw a Notice of Contact Preference. It is also called form 11314E.
