How do I reclaim my traditional Indigenous name?Once you've finished filling out your forms you need to get them . This means you have to swear in front of a commissioner that you've followed all the rules and told the truth in your application. You also have to sign your application form in front of them.
A commissioner can be:
- an Ontario lawyer,
- a provincial judge or justice of the peace,
- a member of the Legislative Assembly of Ontario, or
- a clerk, deputy clerk, or treasurer for a local municipality.
Some ServiceOntario centers can commission your application. You can use the locator to find the one closest to you. You may have to pay a fee for this service.
