3. Apply to the Board
To apply for a hearing with the Consent and Capacity Board, you must fill out and send in a Form A.
It does not cost you anything to do this.
Send the form in by email to ccb@ontario.ca or by fax to 1-866-777-7273.
You can hire a lawyer to help you apply to the Board and go to the hearing with you. Or the Board can ask Legal Aid Ontario to get you a lawyer for free to go to the hearing with you. You can also go to the hearing on your own.
If you’re in a psychiatric facility
The process of applying to the Board is different if you're a patient in a psychiatric ward or facility.
Your must let you speak to a Rights Adviser before you apply to the Board.
Talk to a Rights Adviser
A Rights Adviser helps patients learn about their right to refuse or accept medical treatment.
They do not work for the hospital or . They help you for free. They're also neutral, which means that they cannot tell you what to do. And they cannot tell anyone what you talk about.
A Rights Adviser can:
- talk to you about what your health-care provider wants to do
- help you apply to the Board
- help you hire a lawyer who can go with you to your hearing with the Board
