3. Review and send your application

Make sure you:

• use original documents with the original signature and official seal or stamp from the courts and police
• answer the questions on both sides of the cannabis record suspension application form
• write in block capital letters using blue or black ink, if you fill out the form by hand
• sign the application
• send in the form within 12 months of signing it

Include a photocopy of a government ID that has your name, date of birth, and signature.

Before mailing your application, you should photocopy the original documents and keep a copy for yourself.

You can mail your completed application form, and all original official documents for your Cannabis Record Suspension Application to the Parole Board of Canada at the address:
