2. Fill out the application forms to change your gender marker

To change the gender marker on your birth certificate and birth registration using the adult , you need the following forms:

  1. Form 007-11325e: Application for a Change of Sex Designation on a Birth Registration of an Adult
  2. Form 007-11076e: Request for Birth Certificate
  3. Form 007-11324e: Statutory Declaration for a Change of Sex Designation on a Birth Registration of an Adult.

Download the forms and type in the answers or print the forms and complete them by hand. Do not add an e-signature. The forms must be signed by hand.

1. Application form


Form 007-11325e: Application for a Change of Sex Designation on a Birth Registration of an Adult asks for your contact information. And it has a checklist of the other forms and supporting documents you need to submit with your application.

2. Request for birth certificate form

Form 007-11076e: Request for Birth Certificate asks for information about you, including:

  • your name and mailing address
  • when and where you were born
  • information about your parents

If this is your first birth certificate or a replacement one the gender marker you want on your birth certificate If you do not want a gender marker on your birth certificate, check off the box that says, “I do not want sex displayed on this birth certificate.”

Guarantor 

You need a guarantor to sign Form 007-11076e: Request for Birth Certificate. This is a person who can confirm your identity and the information in your application.

Your guarantor must be a Canadian citizen who has known you for at least 2 years. And they must work in a profession listed on this form.

Some of the people who can be a guarantor include:

  • doctors, surgeons, nurses, pharmacists, dentists, psychologists, or optometrists
  • teachers, principals, or vice-principals in primary or secondary school
  • social workers or social workers
  • professional engineers or accountants
  • lawyers

Changing your legal name 

You do not have to submit Form 007-11076e: Request for Birth Certificate if you apply to change the gender marker on your birth certificate in the same envelope with an application to change your legal name.

ServiceOntario automatically sends you a new birth certificate when you change your legal name. But you still need to submit these forms:

  • Form 007-11325e: Application for a Change of Sex Designation on a Birth Registration of an Adult
  • Form 007-11324e: Statutory Declaration for a Change of Sex Designation on a Birth Registration of an Adult.

3. Statutory declaration 

You need to fill out the top of Form 007-11324e: Statutory Declaration for a Change of Sex Designation on a Birth Registration of an Adult. But you must sign this form when your application package is ready and you're in front of a Commissioner for Affidavits. Read more about that at Step 4.
