How do I change the gender marker on my Ontario birth certificate?When you apply to change the gender marker on your birth certificate you must submit originals of:
- all existing birth certificates,
- birth certificates with parental information, and
- certified copies of birth registration a letter from a practicing doctor, psychologist, or psychological associate
Letter from your health professional
You need to submit a letter signed by a practicing doctor, psychologist, or psychological associate. They must be authorized to practice in Canada and the letter should:
- be on their professional letterhead
- be signed by them
- have their license number
The letter should use the same words as the ones on Form 007-11325e: Application for a Change of Sex Designation on a Birth Registration of an Adult. The letter must say that they:
- are a practicing member in good standing of the appropriate regulatory body, for example, the College of Physicians and Surgeons of Ontario or the College of Psychologists of Ontario,
- have treated or evaluated you (give your full name) who is requesting the change in sex designation (specify the change in sex designation)
- confirm that your does not accord with the sex designation on your birth registration, and
- are of the opinion that the change of sex designation on your birth registration is appropriate.
The letter must include your full legal name. You must submit the original letter not a copy.
