4. Apply for the CDB

To apply for the Canada Disability Benefit (CDB), fill out an application form and send it to Service Canada.

If your legal representative fills out your application for you, they must use the Canada Disability Benefit Legal representative Application. They must also have proof that they're your , for example, a court order or a certificate of guardianship.

You or your legal representative can apply:

  • online, or
  • by phone at 1-833-486-3007. For TTY, call 1-833-467-2700.

Or, you can print out a paper copy of the form and:

  • bring it to a Service Canada office, or
  • mail it to:

What you need to apply

To fill out the application, you need your:

  • social insurance number, and
  • immigration status.

You should also have your banking information if you want to set up direct deposit. This means that the CDB payment goes directly into your bank account.

With a letter from Service Canada

If you got a letter from Service Canada telling you how to apply, you should apply using the 6 numbers on the top right corner of the letter.

Put the numbers where the application asks for your “unique application number”. This links your application to your tax information and may make the application go faster.

Without a letter

If you did not get a letter, you can still apply. But you must give a mailing address. And you should have your most recent tax return, so you can add information about your income.

If you have a spouse or common-law partner, you also need their tax information.

After you apply

Service Canada tries to decide about your application within:

  • 28 days of getting your application if you applied for yourself
  • 49 days of getting your application if your legal representative applied for you

You can call or visit a Service Canada office to find out about your application.

Monthly payments

You start getting monthly payments the month after Service Canada approves your application. If you're getting less than $20 a month, you get the full amount for the year in one payment.

You do not have to reapply for the CDB. But Service Canada reviews your information every year to make sure you still qualify to get the CDB. For example, you must file your income taxes each year.
