3. Gather documents

You need other documents and have to fill out more forms if you're registering a child:

  • with a single name,
  • born to a surrogate, or
  • with 5 or more parents.

Single name

You can register a child with a single name if a parent's culture has a tradition of single names.

You must give proof that the parent's culture has a tradition of single names. For example, you can include a copy of your birth certificate that shows you have a single name.

You must fill out and sign a Request for a Birth Registration with a Single Name Form.

If you register your child's birth online, make sure you fill out this form first. And after you complete the online registration, then mail the form with your proof to the Office of the Registrar General.

If you register by mail, send the form and your proof to the Office of the Registrar General with your Statement of Live Birth form.

Surrogate birth

If your child was born using a surrogate, you have to fill out, sign, and commission these forms:
