What documents do I need to change my legal name?Depending on your situation you may need to submit more documents. For example, you may need to send:
- Photocopies of all change of name certificates or documents if you have changed your legal name before in Canada or abroad.
- Photocopies of all court orders or separation agreements that deal with legal custody or decision-making responsibility for you, if you're 16 or 17 years old.
- A registered mail receipt that shows you sent the Notice form to your partner if you're married, or signed a Joint Declaration of Conjugal Relationship.
Police record check
You need to include an original police record check only if you answered “Yes” to certain questions in your . These are questions 2, 3, 4 or 5 in Part 1, Section D of Form 007-11155E: Application to Change an Adult’s Name.
If you fill out your form online, the system will tell you if you need a police record check based on your answers. And it will guide you on your next steps.
To request a police record check, give the police a filled-out copy of Form 002-5349E: Requirements for a Police Record Check for a Change of Name.
Gender marker
You need to submit more documents if you're also changing the gender marker on your Ontario birth certificate while you're changing your legal name.
Single name
In some cultures, people have single names. If you're changing your name to a single name, you need to include a document to prove that single names are part of your culture. Form 007-11342e: Request for a Name Change to a Single Name lists the documents you can submit to prove this.
