How do I get a health card for my baby?If your baby was born in Canada but not in Ontario, you apply for their OHIP card at a ServiceOntario centre.
You have to fill out and bring Form 0265-82: Registration for Ontario Health Insurance Coverage. And you have to bring documents that prove:
- your identity
- you live in Ontario
- your baby was born in Canada
Document list
Form 9998E-82: Ontario Health Coverage Document List, lists all the types of documents you can use. For example, you can use a rental agreement or property tax bill to prove you live in Ontario. And you can use a passport, credit card, or a valid Ontario driver's licence or Photo Card to prove your identity.
You must bring original documents. Photocopies are not accepted.
If you and your baby had health coverage in another Canadian province, you should also bring your health cards from that province for both of you.
Proof baby was born in Canada
The documents you can use to prove that your baby was born in Canada depend on how soon you apply.
If you apply within 90 days of your baby's birth, bring one of the following:
- a letter from the hospital where your baby was born that has your baby's name, date of birth, place of birth, and the names of their parents
- a Certified Statement of Live Birth from the Registrar-General in that province or territory
If you apply more than 90 days after their birth, you must bring proof of your baby's citizenship. For example, you can bring your baby's birth certificate from a Canadian province or territory or their Canadian passport.
Get your baby’s health card
It usually takes ServiceOntario 6 weeks to process your form and mail your child's health card. You can use the transaction record you got at ServiceOntario as a temporary OHIP card.
