1. Gather your forms and documents

To change your legal name as an adult, you must fill out Form 007-11155E: Application to change an adult’s name.

If you cannot download the form online and print it, you can ask ServiceOntario to mail the form to you.

You may need to fill out and include other forms with your to change your name:

Non-publication form 

Legally, most name changes must be published in the Ontario Gazette. This is an official Government of Ontario publication that anyone can access online. The Ontario Gazette lists a person's previous and new legal names.

If you're changing your name because you're trans or Indigenous, you can choose not to publish your name change in the Ontario Gazette. You must fill out and submit Form 007-11320e: Request for Non-Publication in The Ontario Gazette with your legal name change application. Send this form in the same envelope with your application to change your legal name.

Gender marker on Ontario birth certificate 

If you were born in Ontario, you can also apply to change your gender marker on your birth certificate when you apply to change your legal name.

You can submit both applications, to change your legal name and change your gender marker, in the same envelope. If you do this, you do not have to pay the $35 fee to change the gender marker on your birth certificate later. But if this is your first birth certificate, you have to pay $25.

If you choose to include a gender marker on your new birth certificate, you can use the certificate to update the gender marker on other identity documents. For example, to update the gender marker on your Ontario driver's license.

Single name 

In some cultures, people have single names. If you're changing your name to a single name, fill out Form 007-11342e: Request for a Name Change to a Single Name. Send this form with your application to change your legal name change. You must also send one of the documents listed in the form to prove that single names are part of your culture.  

Supporting documents
