How do I change my legal name in Ontario?To change your legal have to fill out Form 007-11155E: Application to Change an Adult’s Name.
You can download the form and type in the answers. You can also print the form and fill it in by hand. Use blue or black ink and write clearly.
Or you can complete the form as an online guided questionnaire. At the end, you have to download and print the pre-filled forms, and you get a personal checklist.
If you're filling out the form electronically, do not add an e-signature. It must be signed by hand in blue or black ink.
The form has many parts, some of which you may not need to fill out. But you must include all 17 pages when you send your to ServiceOntario, even the parts that are left blank.
Information the form asks for
You must give your current legal name and what you want your new legal name to be.
You must also say why you want to change your legal name. You do not need to fill all the space. For example, if you're changing your legal name because you're trans, you can simply write, “I'm and am changing my name to better fit with my .”
Time in Ontario
You need to write the number of years and months you have been living in Ontario on the day you apply. You must have lived in Ontario for at least the past 12 months before you apply.
Other personal information
Some other information you must include is:
- your date and place of birth
- your contact information
- your marital status and information about your married partner, if you're married
- your parents' legal names
If you were born in Ontario, your information must match what's on your current birth certificate and birth registration.
If you have changed your legal name before, you must also include details such as the date of your last name change, what you changed your name from and to, and where you changed your name.
If it applies to you, you must also give information about:
- any criminal charges, convictions, or findings of guilt
- any non-criminal legal cases against you, for example, family law cases, or traffic violations
- any outstanding law enforcement orders against you, for example, a warrant or restraining order
- certain financial information, for example, any unpaid court or tribunal debts, bankruptcies, and loans
You must answer all questions honestly. Saying “yes” to any of these questions does not automatically mean that your name change application will be rejected.
Gender marker on birth certificate
You can say if you do not want a gender marker on your Ontario birth certificate. If you choose this option:
- it does not change the gender marker on your birth registration, and
- you cannot use the birth certificate to update your gender marker on other documents.

Fees
You can pay by attaching a cheque or money order. To pay by credit card, provide your credit card information.
Some organizations that help people change their legal name may cover the cost of your application.
