5. Sign and submit your application

You must sign the Statutory Declaration in Part 6 of Form 007-11155E: Application to change an adult’s name in front of a Commissioner for Affidavits.

If you're applying to change your gender marker on your Ontario birth certificate at the same time, you need to fill out and sign another form in front of a commissioner. It's Form 007-11324e: Statutory Declaration for a Change of Sex Designation on a Birth Registration of an Adult.

When your is ready, take it with all your supporting documents to a Commissioner for Affidavits. After you sign your forms, the commissioner signs and fills out the bottom of the forms.

A Commissioner for Affidavits is someone who has the power to sign your affidavits or declarations. They do this after having you swear or affirm that what's in a document is true. A lawyer, paralegal, municipal clerk, or notary public, can be a Commissioner for Affidavits.

Where to find a commissioner

You can commission your documents at some ServiceOntario locations. Using their online tool, search “Commissioner of Oaths” to find locations that have a commissioner and book an appointment. Call before you go and ask if there's a fee.

Some community legal clinics and organizations that help people change their name on identity documents may commission your application for free.

Or you can pay a commissioner, like a lawyer or notary, to commission your documents for you. It usually costs between $10 to $30 to commission one document.

Submit your application 

It's a good idea to photocopy your complete application for your records before you submit it.

Include all your forms and documents, and your fee payment in the same envelope. Write your address on the top left corner of the envelope.

Use registered mail so you have proof it was delivered. You can also submit your application in person at:

If anything is missing from your application, ServiceOntario will contact you or return the package with instructions.

It usually takes ServiceOntario about 6 to 8 weeks to change your legal name. It can sometimes take longer. You can check the processing time online.
