What if I already paid an illegal charge or rent increase?
In Part 2 of the form, you should choose Reason 1 if you paid an illegal rent. You should choose Reason 2 if you paid any other illegal charge. For each reason, you must give details about the payments you made and how much you think the landlord owes you.
There is a large box for you to give more details about what happened and why you think the charges were illegal. You can add extra pages if this is not enough room.
