1. Fill in the reconsideration form

You must use the Request for Reconsideration form.

You can get it from any Service Canada office. Or you can find it online by typing INS5210 in the search box on any Government of Canada website.

You can ask for a if Service Canada:

  • refused to give you benefits or you think the amount is too low,
  • told you to repay some benefits you received,
  • gave you a penalty, or
  • gave you a warning letter.

There's no cost to ask for a reconsideration.

Timelines

Make sure Service Canada gets your reconsideration request within 30 days from the day you found out about the decision you disagree with.

If you miss this deadline, you can ask for more time. But you must give a good reason why you were late.
